all the slots are on seperate tabs 1to8 which you switch between using the joypad triggers or numbers 4&5 on the keyboard/keypad. so, you get 8 pages of slots with 12 slot spaces each. 96 in all? the 4 mutagens on the first page are active/the same for all 8 tabs. i cant actually say if all skills were active but i think they were.
